Nitpick: technically a VST2 instrument is literally the same thing as an effect, except that it sets one bit(!) to indicate that it’s an instrument.

For compatibility reasons I’d suggests having outputs on instruments and both inputs and outputs on effects (some hosts might get confused otherwise), but I don’t think they are technically required and as far as I’m aware having inputs on instruments usually won’t hurt (though whether any given hosts can make use of them varies).
